How to complete LGV / PCV Provisional forms

1) A GP must complete D4 Medical Report - there is a charge. Contact your doctor or see our list above.

2) SECTION 1: Why not tick all boxes for all provisional entitlements you may need others in the future

C   - for rigid lorries up to 32 tonnes (old HGV 2)
C1 - for smaller lorries up to 7.5 tonne (C1+E is 7.5 + trailer)
(You can’t apply for LGV C+E (for drawbars & articulated vehicles’) until you’ve obtained LGV C licence
D   - for buses and coaches.  Tick D1 if you only want a provisional for a minibus. 

3) SECTIONS 2 & 3 complete with all your details & eyesight and hearing details (and organ donation if you wish) and details of any driver hours or roadworthiness and non-driving convictions (not driving endorsements).  Then turn over the form to complete SECTION 4 and other sections.

4) SECTION 5 & 6 – Your proof of identity / Signing a photo – READ NOTE ‘B’ which is attached to the D2 form for help, but you probably won’t need to fill Section 6 if you already have a photocard driving licence.

5) Section 7, REMEMBER – KEEP YOUR SIGNATURE INSIDE THE WHITE BOX!!!

No Photocard licence?  After your medical complete D2 form & take your ID to a main Post Office with £4 for their Premium Service or local DVLA office (Wimbledon & Boreham Wood).  They check your forms and return your passport/ ID and then send everything to DVLA for you. 

Got a Photocard licence?  After your medical send D2 & D4 forms & both parts of your licence to: DVLA, Swansea, SA99 1BR - (If you don’t need a new photo don’t attach one, as change of photo confuses DVLA and delays the return of your licence)

Need provisional /return of your car licence quickly? Visit your local DVLA office (Wimbledon or Boreham Wood) for a counter service
DON’T SEND MONEY WITH YOUR APPLICATION – YOUR PROVISIONAL IS FREE
Unless you exchange a foreign licence to a UK licence at the same time which is £50  

Your new Licence and Counterpart, which shows your provisional, will be sent to you after about a week (together with ID, if submitted)
